You must do a self-check using this form ANYTIME BEFORE coming on to our school grounds COVID Employee Self-Check Health Screening Form.

There will be a point where someone will be doing a temperature screen/COVID check and you will be directed to be screened by this person before going to different areas of the school grounds.
